CNA Certification In New York

With the growing need for certified nurse aides, applicants are required to receive CNA certification in New York after completing the program approved by NY Department of Health and qualifying the competency examination. After passing the CNA exam, the candidate’s name will be placed on the New York State Registry, which is maintained by the Prometric.

Requirements to Obtain NY Licensure

1. New Nurse Aide

If you have never certified and worked as a nurse aide, you must fulfill the following eligibility criteria:

  • Successfully complete a nurse aide program program approved by the New York state within the past 2 years.
  • If you are presently working in any NY nursing home, request your employer to fill all the details mentioned in Section 2 of the application form.
  • Successfully pass the written or oral exam and the clinical skills exam within 24 months of completing your program course.

2. Reciprocity / Transferring CNA Certificate from Another State

If your name is listed on the nurse aide registry of another state and wish to become certified in NY, you must:

  • Submit a copy of your current CNA license.
  • If the name mentioned on your CNA certificate is different from your current name, provide evidence verifying name change such as marriage certificate.
  • If your current state of licensure does not provide expiration dates on its registry, you are required to submit proof of employment. This documentation must be provided on facility letterhead from your employer and indicate a working experience of 7 hours (paid), in the last 2 years. The letter must be signed by your supervisor and must also signify that you have performed nurse aide duties.

3. Graduate Nurses

If you are a trained graduate nurse in the United States, you are not required to complete any further program, but you will have to take the exam.

You need to submit the below-given documents to make yourself eligible for the exam:

  • Completed application form
  • Copy of nursing program completion certificate

You are also required to be:

  • Permitted by the New York State Department of Health (NYSDOH) to take the exam.
  • Provide the approved application with required fee.
  • Successfully pass the written or oral exam and clinical skills exam within 24 months of submitting your application.

4. RNs and LPNs Certified in the United States

If you currently hold an active LPN or RN license in the United States, you are not required to take any exam or complete any further program. You must:

  • Submit a copy of your current and active LPN / RN license.
  • If you are working as a CNA in a nursing home in NY, request your employer to complete Section 2 of the application form.

5. Foreign Trained Nurses

If you are a foreign-trained nurse, you must receive approval from the NYSDOH. Submit the following documents:

  • A photocopy of your Social Security card.
  • A photocopy of your nursing license.
  • A photocopy of your nursing school diploma.
  • If you are working as a nurse aide in a nursing home in New York, you must request your employer to fill the required information in Section 2 of the application form.
  • A copy of your score report or admission letter if you have already taken or will appear for the New York State RN or LPN examination.
  • Provide documents of your nursing school coursework.

You are required to submit your application and documents to:

Besides this, you are also required to fulfill all the criteria mentioned above (See Requirements to Obtain NY Licensure – Graduate Nurses).

6. Trained and Lapsed

Your license is considered as lapsed, if you are not employed as a paid CAN, in the past 2 years at a nursing home or any other facility approved by the state of New York. If your certification is lapsed and you have completed an approved program on or after July 1, 1989, you must:

  • Successfully pass the written or oral exam and the clinical skills exam within 24 months of application submission.
  • If you are currently working as a CNA, ask your employer to fill Section 2 of the application.
  • Provide NYS certification number on the application.

7. Lapsed – Other

If your CNA license is presently lapsed and had been achieved through waiving, deeming, reciprocity, or if you completed your nurse aide program before July 1, 1989, you are required to:

  • Complete reprogram. Request your program program coordinator to fill all the details of Section 3 of the application form.
  • Other requirements are same as mentioned above (See requirements of Trained and lapsed).


Procedure to Obtain the CNA Certification in NY

The step-by-step procedure to complete certification in the state of New York is as follows:

Route 1 – New Nurse Aides
Route 2 – Reciprocity / CNA from another State
Route 3 – Graduate Nurses
Route 4 – RNs and LPNs licensed in the U.S.
Route 5 – Foreign – Trained Nurses
Route 6 – Trained and Lapsed
Route 7 – Lapsed or Other

Step 3 – Submit the non-refundable fee(s) in the form of certified check or money order made payable to “NY Commissioner of Health, NYNA.”

Step 4 – Submit the paper application to:

How to Check the Application Status?

Candidates who have applied for CNA license through Prometric may check the status of their application by clicking the below link:

If you have submitted online application, status is generally available within 24 hours. While in paper application, it may take up to 7 business days after the application was mailed to the Prometric.

You will be required to enter the following details:

  • Prometric ID: a unique number located on your application status report, authorization to test letter, online dashboard (if applied online), or score report.
  • Last Name (name mentioned on the application)
  • Birth Year
  • Program

How to Renew NY License?

All nursing assistants must renew their CNA certificate in every 2 years. Use the following link to download the recertification application:

Verification of CNA License in NY

Use the following link to search for the certified nurse aides in the New York State Registry:

Applicants may complete the search process in either ways:

  • By certificate ID, or
  • By name

Change Request (Name, Address or Duplicate License)

Use the below given form to update any changes in the Prometric registry files:

Applicants may also request for a duplicate license only in states where the registry is managed by the Prometric. A fee of $15 is charged and the mode of payment varies with the states.

For name change, you must provide a copy of legal documents such as divorce decree, marriage certificate, driver’s license, legal name change document, or SSN card.
