Renewal / Recertification of the CNA License

CNA Certification Renewal Requirements

  • To renew your CNA license, you must have rendered at least 8 hours of paid nursing services in a specialized facility, licensed nursing facility, continuum of care facility, home health or home care agency, residential care home, assisted living center, adult day care center, Oklahoma Department of Veterans Affairs nursing facility, or Oklahoma correctional facility.
  • Attach a completed “Affidavit of Lawful Presence” along with the recertification form.
  • The proof of working hours must be within a period of 2 years.

Question 1 – Can I e-mail or fax my license application form to the Oklahoma Nurse Aide Registry?
